BOTOX…..It’s not just for wrinkles anymore.

Botox has been around now for 15 years and has been a game changer in the world of wrinkle smoothing.  Basically it partially paralizes the muscles in the forehead and around the eyes giving marked relaxation to those muscles and an improvement or elimination to the wrinkles.  It has also been used in kids with cerebral palsey to relax the contractured muscles seen in that unfortunate condition to allow physical therapy to the affected muscles.  Next came a wonderful breakthrough in the treatment of migraine headaches where Botox can be injected in the “trigger” muscles in the scalp, back of the neck or other areas giving marked relief of the headaches in both severity and recurrance.  TMJ syndrome is also treated quite well with injection into the Masseter or chewing muscle which greatly reduces or eliminates the teeth grinding at night.  Tired of those pucker lines around the mouth or that bulging chin?  Botox injections properly placed can reduce those lines and also give your chin a more tapered normal look too.  Injeciton into the “neck bands” and neck wrinkles is showing promise for a more youthful neck.

Over the last several years more and more innovative uses for Botox have immerged that are proving once again to be game changers in medicine.  Many patients present with a wide face from a thickened Masseter muscle.  Injection into that muscle (the chewing muscle) tends to relax it and “thin” its profile so that the face has a slimmer profile without doing any surgery.  An amazing treatment we do at Genesis is injection of Botox to reduce excessive sweating known as HYPERHYDROSIS.  In this condition excessive sweating occurs even if you are in a cold room, giving you that ugly ring under the arm.  It also occurs in the palms of the hands and soles or the feet.  Injections of Botox into those areas relaxes the small muscles in the sweat glands so that less sweat is secreted.  Now no more embarassing sweat rings on your shirt or dripping wet hand shakes.  Best of all while most cosmetic Botox treatments wear off in 3-4 months, Botox for headaches and hyperhydrosis tends to last 5-8 months in most cases.

An article was written a couple years ago and new research this year has shown an association with Botox usage and an improvement in depression.  While not a cure for depression the newest research is promising for those with mild depression.

Botox can be combined with facial fillers such as Juvederm so the combination gives a more youthful “lifted'”face by working in synergy on the wrinkles and the volume loss in the face.

Botox and/or fillers….what’s the difference

Patients often ask me which they should get on their face… Botox or Juvederm filler.  My answer is….both if you need it.  However, they are usually mutually exclusive as Botox is used to reduce forhead, glabella, and crow’s feet wrinkles by muscle paralysis and Juvederm products are used to fill in spaces on the face where there is lost volume. A recent article in Cosmetic Surgery Times discusses that overlaping the Botox with the filler can slow down the absorption of the filler, which usually happens over about a years time period.  They found that decreasing the activity of the muscles in the face does tend to slow the metabolic absorption of the filler.  Unfortunately, fillers should be used with extreme caution in the Glabella region (the area of the forehead  between the eyes) because over usage there can potentially cause some blood vessel occlusion and tissue death.  That area is a great place for the Botox as it softens the muscle that causes those hideous”11’s” that make you  look angry all the time.  By the same token filler looks great to plump the lips but if you use Botox in the lips then your mouth may look weird due to the paralysis of the muscle around the mouth (Obicularis Oris).

Now I can inject Botox and Juvederm at the same visit in their needed places.  Where I think that using both in the same area is good is on that patient with deep crows feet that may only have a partial response to the Botox.  This indicates to me that despite paralyzing the muscle under the crows feet, the skin is loose from sun damage and volume loss.  A carefully placed small amount of Juvederm filler on top of the Botox will help tremendously to smooth and fill the area while the Botox will help slow down the absorption of the Juvederm.   Overlap of Botox and filler in very deep forehead wrinkles has also done well especially if depressed scars are present from previous injury.  Overall, we can make your face look years younger with our Botox and Juvederm filler injections.

Fat transfer or Juvederm, which do I do…Part 1

At Genesis we offer two unique methods to fill in those sagging and hollowed places on the face and hands. Today we will talk about fillers, particularly JUVEDERM products.

For years fillers such as Juvederm, Radiesse, Sculptura, and others have been on the market with millions of patients worldwide using them.  At Genesis we are using the complete line of Juvederm products including the one introduced about 2 years ago called VOLUMA.  I like the Juvy products in that it gives me a wide range of thickness of products depending on where in the face they are used.  For instance we would use the thickest product, Voluma, under the eye and inject it against the orbit bone to build up that caved in area,  making your cheek look fuller and more youthful.  This is a very common area that we see even in younger patients.  If you or your friends think you look tired all the time, and you’re not, it may be due to the volume loss under the eye.  The mid-grade thickness of Juvederm is ideally suited for the folds against the nose and the “marionette” lines below and around the mouth.  The thicker Voluma is not designed for that area and may clump badly if injected there.  The thinnest Juvy is great for lips and also around the mouth and nose area too.  In some people, especially older, I may layer it over the Voluma that I placed under the eye to acheive an even better lift and taper effect.  I can also use it in those “pucker lines” or “smoker lines” that develop in the upper lip.  This is done by thinning the Juvy a little more and injecting each of those troublesome lines individually.  The effect is instantaneous and the compliments are too.

BOTOX ….Look ma no wrinkles….or headache

Let’s get those wrinkles under control in 2016 and Botox is a great way to do it.  Today we are going to talk about the many uses and successes of Botox.  Botox has been around now for well over 10 years.   Its use has grown from treating those wrinkles on your face and that always angry look between your eyes to multiple medical uses.  Botox works uniquely to partially paralyze the muscles in which it is injected.  Contrary to urban legend, in cosmetic doses it does not migrate elsewhere and render you paralyzed.  I promise.  Botox is manufactured under strict controls and when used as directed is extremely safe and effective.  Other products are on the market including Dysport and Xeomin but I have not found them to be as reliable or long lasting as Botox.  Therefore, I am sticking with Botox.  As I stated the cosmetic use in the forhead and around the eyes has been here for well over 10 years.  Now the FDA has approved the use of Botox to treat headaches.  I have several patients in which injection in the back of the neck, the temporal area on the side of the head, and the area above the hairline have given them marked relief from tension and migraine type headaches.  We have also treated TMJ syndrome with it injected at the bottom of the chewing muscle on the jaw.  One patient went from 20+ headaches a month to one with forhead and hairline injections.

One recent article now has shown that patients with depression have shown improvement with Botox treatments.  The mechanism of its success is being investigated but reapeat studies have confirmed this finding.
