Think Combination Treatments for facial lifting

 

 

Want to look your best? Are your tired of the facial wrinkles and the sagging skin face? You don’t want a face-lift that requires some cutting but you just want to look refreshed and more youthful.

 

The most trending treatments for minimally invasive to non-invasive facial rejuvenation has been to combine several old line therapies at once to achieve a more youthful face and neck appearance. For many years we have used Botox to smooth wrinkles on the forehead and around the eyes and have used dermal fillers such as Juvederm to make the face look fuller by replacing lost facial volume. Strategically placing both Botox and fillers selectively can take years off your face. Juvederm and other filler brands now have different thicknesses of their products that can be targeted for a specific area of the face. A thinner filler is targeted for the lips but can also be used to fill in the “smoker lines” that appear in the upper lip and corners of the mouth. At the same time we can put slightly thicker filler deeper to the area around the mouth to fill in where volume loss may have given you a sunken appearance around the mouth. In cases of severe mouth corner downturn, or a permanent frown, Botox can be used to relax the selective muscles that cause the problem. What I notice on so many patients is that there may be some slight jowling along the jaw edge that gives the appearance of a sunken place between the jowl and the chin. By carefully placing thicker filler along the jaw edge in that area the line from the jaw edge to the chin can be evened out and give you a more youthful jaw curve. If your chin is too flat or misshapen it is possible to combine Botox to relax the chin muscle and a small amount of filler to shape the chin to fit your jaw contour. A little further up from the mouth edge filler can also be used to smooth and fill out the naso-labial fold that gives your face the parentheses look. It doesn’t take a lot of filler to do this but by placing it properly the area from the nose to the chin can be transformed to a younger, fuller appearance.

Similarly the eye area is another place that can be transformed using fillers and Botox. As you age, volume loss under the eyes can make you look tired all the time, even when you are not. In severe cases I use Voluma filler under the eye to replace that lost volume and trim years off your face. In less severe cases a less dense grade of Juvederm can be used. Again, it is placed to not only fill in but to lift the skin around the eye and improve the saggy sad look. Combine that with Botox on the “crows feet” and a new look can be easily accomplished. I now have several patients who are getting Botox on the neck to relax those wrinkles caused by aging and sun. While it won’t give you a completely smooth neck it can greatly flatten and improve your appearance.

After filling in the volume loss areas and flattening the most severe wrinkles with Botox, the skin can be further treated with laser resurfacing or Renuvion facial treatment to thicken the collagen support of the skin and further tighten the finer lines and surface problems. Adding Platelet Rich Plasma (PRP) to the resurfacing can promote rapid healing and a more youthful skin.  Additionally, placing a collagen solution on the face during our Hydrofacial can return a beautiful glow and plumpness to the skin. By combining all these treatments from deep to superficial you can achieve a more youthful appearance for the holidays and years to come.

BOTOX…..It’s not just for wrinkles anymore.

Botox has been around now for 15 years and has been a game changer in the world of wrinkle smoothing.  Basically it partially paralizes the muscles in the forehead and around the eyes giving marked relaxation to those muscles and an improvement or elimination to the wrinkles.  It has also been used in kids with cerebral palsey to relax the contractured muscles seen in that unfortunate condition to allow physical therapy to the affected muscles.  Next came a wonderful breakthrough in the treatment of migraine headaches where Botox can be injected in the “trigger” muscles in the scalp, back of the neck or other areas giving marked relief of the headaches in both severity and recurrance.  TMJ syndrome is also treated quite well with injection into the Masseter or chewing muscle which greatly reduces or eliminates the teeth grinding at night.  Tired of those pucker lines around the mouth or that bulging chin?  Botox injections properly placed can reduce those lines and also give your chin a more tapered normal look too.  Injeciton into the “neck bands” and neck wrinkles is showing promise for a more youthful neck.

Over the last several years more and more innovative uses for Botox have immerged that are proving once again to be game changers in medicine.  Many patients present with a wide face from a thickened Masseter muscle.  Injection into that muscle (the chewing muscle) tends to relax it and “thin” its profile so that the face has a slimmer profile without doing any surgery.  An amazing treatment we do at Genesis is injection of Botox to reduce excessive sweating known as HYPERHYDROSIS.  In this condition excessive sweating occurs even if you are in a cold room, giving you that ugly ring under the arm.  It also occurs in the palms of the hands and soles or the feet.  Injections of Botox into those areas relaxes the small muscles in the sweat glands so that less sweat is secreted.  Now no more embarassing sweat rings on your shirt or dripping wet hand shakes.  Best of all while most cosmetic Botox treatments wear off in 3-4 months, Botox for headaches and hyperhydrosis tends to last 5-8 months in most cases.

An article was written a couple years ago and new research this year has shown an association with Botox usage and an improvement in depression.  While not a cure for depression the newest research is promising for those with mild depression.

Botox can be combined with facial fillers such as Juvederm so the combination gives a more youthful “lifted'”face by working in synergy on the wrinkles and the volume loss in the face.

Young Hands…Fat transfer for youthful hands

Patients spend a lot of time and money to make the face more youthful and vibrant.  But what about your hands.  So many times others look at your face and may think you are at least 10 years younger than you are….until they see those hands with the pigment spots and the volume loss that exposes the tendons and veins.  Genesis can help give you that youthful hand you need.

First of all let’s address the pigment spots and wrinkled skin.  We can employ our Fraxel laser technology to help tighten the skin and fade those pigment spots.  While Fraxel is used mainly on the face and neck, the same results can be achieved on the lower arm and hand.  The area is numbed with a cream before starting and only takes a few minutes per side.  We do recommend 4 treatments spaced a month apart to get that firmer skin and youthful look.

What has really turned back the clock on hands is the use of fat transfer.  While we can do the transfer just to the hands most of our patients also want the face done also to lift the cheeks and treat around the mouth.  Basically, we can harvest fat from the abdomen or leg, mix it with your own platelets then place it between those unsightly tendons and knobby knuckles, giving you a smooth hand.  The prominent veins that are seen with aging are also hidden and more natural looking.  Because of the platelets mixed in with the fat, some skin toning may occur due to the action of the platelets.  The fat is massaged in place after being injected to give a smooth homogeneous appearance.  It really looks great.

Botox and/or fillers….what’s the difference

Patients often ask me which they should get on their face… Botox or Juvederm filler.  My answer is….both if you need it.  However, they are usually mutually exclusive as Botox is used to reduce forhead, glabella, and crow’s feet wrinkles by muscle paralysis and Juvederm products are used to fill in spaces on the face where there is lost volume. A recent article in Cosmetic Surgery Times discusses that overlaping the Botox with the filler can slow down the absorption of the filler, which usually happens over about a years time period.  They found that decreasing the activity of the muscles in the face does tend to slow the metabolic absorption of the filler.  Unfortunately, fillers should be used with extreme caution in the Glabella region (the area of the forehead  between the eyes) because over usage there can potentially cause some blood vessel occlusion and tissue death.  That area is a great place for the Botox as it softens the muscle that causes those hideous”11’s” that make you  look angry all the time.  By the same token filler looks great to plump the lips but if you use Botox in the lips then your mouth may look weird due to the paralysis of the muscle around the mouth (Obicularis Oris).

Now I can inject Botox and Juvederm at the same visit in their needed places.  Where I think that using both in the same area is good is on that patient with deep crows feet that may only have a partial response to the Botox.  This indicates to me that despite paralyzing the muscle under the crows feet, the skin is loose from sun damage and volume loss.  A carefully placed small amount of Juvederm filler on top of the Botox will help tremendously to smooth and fill the area while the Botox will help slow down the absorption of the Juvederm.   Overlap of Botox and filler in very deep forehead wrinkles has also done well especially if depressed scars are present from previous injury.  Overall, we can make your face look years younger with our Botox and Juvederm filler injections.

Platelet Rich Plasma…Magic in your blood

Many of you may not have heard of Platelet Rich Plasma (PRP) but it is one of the most phenominal natural substances known to promote healing and enhance results in cosmetic treatments.  So what is it?   Simply your blood has 3 types of cells….The red cells carry the oxygen to the tissues and take away CO2,  the white cells fight off infections and help with your immunity, and the platelets are the powerful healers in the blood.  Anytime you have an injury such as a cut, abrasion or a bruised area the platelets are rushing in to direct the clean-up and healing processes.  They direct the fibroblasts to form new skin and support tissues i and direct the stem cells to make new blood vessels and other necessary tissue.  That’s where we use them in cosmetics.

After drawing your blood we spin it down in a special tube that separates the red/white cells away from the platelets.  The platelets are then harvested and reconcentrated in your serum to give you a more potent platelet mixture.  We then activate the platelets using a Calcium solution which makes them ready to “work”.  These activated platelets can be injected into the scalp to stimulate hair growth or can be injected along with Juvederm fillers to enhance the skin and make the filler last longer.  I have even put PRP on thin non-healing leg skin  and got healing in 3 weeks.  The main thing we use it for is on fat transfers.  These activated platelets when mixed with your fat stimulate millions of stem cells that go on to make new vascular supply and structural support to the newly injected fat. This gives you a better look and more longevity with the transfer.  Injecting PRP under old acne scarring is also very successful to erase the “pot marks” that are left behind.
